Current Services 

Initial Psychiatric, Medical, and Addiction Evaluation: $220.00   

This will include a full evaluation of past mental health diagnoses and treatment. It will also include the attainment of a comprehensive substance, medical, and social history as well as a review of anything which may have brought you to treatment at this time 

This is the standard rate not inclusive of lab testing or additional services

Routine Follow Up Visit: $120

This is a follow up visit following a new patient evaluation visit with myself. It will include medication management and review of therapeutic interventions and techniques which may be of benefit. We will additionally review any new medical changes or new medicinal interventions being provided by adjunct providers. 

(Visits for longer then 30 minutes or including a caregiver/other involved persons, or additional services such as lab/records review will be charged accordingly. Standard rate is 120.00

AFTER HOURS VISIT: Minimum fee based on time and services provided are $175.00

This is an after standard business hours visit, such as Saturday or Sunday at the providers discretion. At times insurance companies will not reimburse for this type of service. However, if your insurance is in network, we will make every attempt to collect from your carrier. Please note that these visits will be infrequent and only at the availability and discretion of the provider. 

Additional Services:

TELEPHONE CONSULTS

This is a phone call from patient in between our scheduled visits. Brief communication of needs or questions is of no cost. However, if this develops into a phone consultation there will be a 120.00 fee which is not generally covered by insurance and paid at the time of service

COMPLETION OF FORMS/DOCUMENTATION

This service does not require scheduling but please keep in mind that this service may require time for completion and return, 

All forms are at the discretion of the provider. 

   Forms will have separate fees based on the time required for completion and potentially the gathering of other records from alternative providers. 

   If it is jointly decided that we are going to pursue a medication which your insurance does not wish to cover, or provides limited coverage for, we will discuss the completion of patient assistance forms. Once determined that you would like to pursue this there may be an additional fee. Please note that prior to submission you will have to have provided all necessary patient information such as financial documents required by the pharmaceutical company. 

   FMLA completion of forms are 75.00 when appropriate. Disability forms (at providers discretion) will have a charge of 150.00 due to the time necessary for preparation and will only be considered once you have been with myself for no less then 3 months to better determine level of capacity. 

Please note that completion of documentation does not guarantee the outcome that you may desire. 

Copy of medical records approved for distribution 1.25 per page and are released at the discretion of the provider. 

These fees are not covered by insurance and they will require payment in advance

Late cancellation and No Call No Show fees are as follows: 

Cancellation of routine follow up visits within the 24 hours prior to a routine scheduled visit will be charged a 50.00 fee. Those meeting the criteria for New Patient appointments will be charged a 75.00 fee. Following 2 missed visit appointments, barring extenuating circumstances as determined by provider, the patient will be potentially let go from the practice. Please note that these fees are not payable by insurance. There is a 75.00 insufficient funds fee for all returned methods of payment
